The professional development hours (PDH) activity log — ENG
4012 (Rev. 04/2006) — is to be used for keeping track of
your professional development hours required for P.E. and P.S. registration
renewal. Use as many copies of the form as needed to maintain an accurate
record of your PDH.
This form does not replace copies of your certificate(s) of completion
or other documentation required by Ohio Revised Code section
4733.151.
Fifteen PDH are required for your annual registration renewal. A maximum
carryover of 15 PDH are allowed per year. For information about Ohio’s

FAQ page.
Activities and courses taken must be relevant to the practice of engineering
or surveying and must include technical, ethical and/or managerial
content.
The professional development hours activity log is provided for your
use and must be submitted to the Board only when
requested by the Board. |
